Information for people interested in rehabilitation stay
If you are interested in a rehabilitation stay in our facility, it is necessary to send the following to the attention of S. Černá:
- Filled-in Questionnaire for applicant for a rehabilitation stay (you can find the form in the section “Download documents”)
- Up-to-date medical report

Information about hospitalization
Your hospitalization planned in advance will start in the receiving office, where you will fill in the forms required for receiving. Office staff will accompany you to the department where you are to be hospitalized. The clinic offers one- to three-bed rooms with above-standard equipment (with own sanitary facility, TV, some also including telephone and refrigerator). The clinic is located in a peaceful part of Prague with the possibility of parking and adequate privacy.
Do not forget to take the following with you:
- Identity card
- Health insurance card
- Sick note (if you have any)
- Name and address of the general practitioner
- Telephone number of the closest relative
- Report about previous hospitalizations and visits to out-patient physicians
- X-rays, examination results (CT, MR, EEG, EMG) – if you have any
- Orthopaedic aids that you use permanently
- Pyjamas (night-suit), dressing gown
- Underclothes
- Personal hygienic items
- Towel
- Slippers
If you use water treatment procedures, do not forget to take the following with you:
- Swimsuit
- Bath towel, towel, dressing gown
- Antiskid shoes (to the swimming pool)
You will need the following for rehabilitation:
- Sportswear (training suit, T-shirt, shorts)
- Adequate sport shoes
